

Every Early Intervention case eventually ends — usually as a child approaches
age 3 and transitions out, or when the case is closed for another reason. Tiny
Solutions tracks both.

## Transition (around age 3) [#transition-around-age-3]

Children approaching age 3 appear in the **Transitioning** view of the
[Children list](/docs/children/overview). On the child's Child information tab,
the **transition** section lets you record the transition plan and dates as the
child prepares to move on to the next program.

## Discharge [#discharge]

When a case closes, an admin selects **Discharge** on the child's record and
records the reason and date. The child then appears in the **Discharged** view.

A discharged record becomes read-only, with a summary banner at the top showing
when and why it was closed. The record stays in your agency for your records — it
is not deleted.
