What is a service log?

The service log is the parent-signed record that a session occurred. Typed names do not satisfy the parent signature. Evaluations and service coordination do not use this log.

The service log is the family's signed confirmation that the session happened. New York expects a handwritten or otherwise accepted parent signature on the log. A typed name is not a signature. The provider's session note cannot stand in for this page.

New York City also expects the parent to sign contemporaneously on site. Completing the log later from memory, or emailing it as the primary path, fights that rule. The handed-over device is the designed moment: one task, then the device goes back to the provider.

Evaluations and service coordination do not use a parent-signed service log. Those services still keep their own documentation. Do not invent a log to make every visit look the same.
