The lifecycle

  1. What is a referral in New York Early Intervention?

    A referral is the communication that a child under three may need Early Intervention. In New York it lands with the Early Intervention Official and starts the 45-day clock.

  2. What is a Multidisciplinary Evaluation?

    The Multidisciplinary Evaluation is the team evaluation that informs eligibility and the first Individualized Family Service Plan. Two or more qualified personnel from different disciplines assess the five developmental domains.

  3. How does eligibility work in New York Early Intervention?

    A child is eligible through a diagnosed condition with a high probability of delay, or through a documented developmental delay that meets New York's thresholds. The written determination is part of the child's record.

  4. What is an IFSP?

    The Individualized Family Service Plan is the written plan for an eligible child and family. It lists outcomes, early intervention services, and the Ongoing Service Coordinator. It is not a school-age special education document.

  5. What is a service authorization?

    A Service authorization is the municipality's permission for one service, to one provider, for a stated period. New York does not issue a separate notice object.

  6. What is a session note in Early Intervention?

    A session note is the rendering provider's contemporaneous record of one session. New York requires it in addition to the parent-signed service log.

  7. What is a service log?

    The service log is the parent-signed record that a session occurred. Typed names do not satisfy the parent signature. Evaluations and service coordination do not use this log.

  8. What is transition in Early Intervention?

    Transition is the planned move out of Early Intervention, most often toward preschool special education as the child approaches age three. It is a required part of the Individualized Family Service Plan.
