What is a session note in Early Intervention?

A session note is the rendering provider's contemporaneous record of one session. New York requires it in addition to the parent-signed service log.

Every session produces two documents. The session note is signed by the rendering provider and records the recipient, the date, the service type, start and end time, progress toward the IFSP outcome, and the provider's name, title, signature, and credentials. The service log is the parent-signed companion. They are not interchangeable.

The note must be written close to the session. A later rewrite that pretends to be contemporaneous is not the same record. When a supervisor must co-sign, New York's qualified-personnel rules set who that is and how long the co-sign may wait. The 45-day co-sign clock is a supervision rule, not the referral-to-plan clock.
