Transitions & discharge

Handle a child approaching age 3 and close a case when services end.

Every Early Intervention case eventually ends — usually as a child approaches age 3 and transitions out, or when the case is closed for another reason. Tiny Solutions tracks both.

Transition (around age 3)

Children approaching age 3 appear in the Transitioning view of the Children list. On the child's Child information tab, the transition section lets you record the transition plan and dates as the child prepares to move on to the next program.

Discharge

When a case closes, an admin selects Discharge on the child's record and records the reason and date. The child then appears in the Discharged view.

A discharged record becomes read-only, with a summary banner at the top showing when and why it was closed. The record stays in your agency for your records — it is not deleted.
